These forms are to be completed and faxed to Brenda Chupp two weeks prior to start of clinicals (at 770-918-3116). This will allow sufficient time for our IT department to process forms.

Faculty:

·         There is no need for a formal orientation with Brenda Chupp. That material can now be accessed via the hospital website at http://rockdalemedicalcenter.org/StudentOrientation2.aspx . Once the paperwork has been completed, please submit to Brenda Chupp.
